Your Blue Badge may not be valid if the information we hold is not kept up to date.
You must tell us if:
- there has been a change of name, address, phone number or email address
- the Blue Badge holder has died
- the badge has been lost, stolen or damaged
For change of address, you need to send proof of your new address. This could be an official document such as a utility bill, Council Tax statement or Department of Work and Pensions (DWP) award letter.
For a change of name, you must send proof of your new name, such as marriage certificate or Deed Poll.
It is important that your contact details are accurate, so that we can issue your renewal advice correctly.
