Supplementary information form

In addition to applying on the Local Authority (LA) application form, this school may require you to complete a Supplementary Information Form (SIF). Please note this form is ONLY required if you have been regularly attending church for at least two years.

These Supplementary Information Forms seek information which is not collected on the LA application form, but which is needed by the school to enable them to reach a decision on your application. 

•  If applying for a 2026 reception place (your child is starting school at 4 years old) please visit our website to complete an online SIF.
• If applying for an in-year place (your child is changing schools) please contact the school for a copy of the SIF. This SIF should be returned directly to the school via post or handed in (not emailed) and not to the Local Authority.

A SIF will not be considered on its own. You must also include the school as a preference on the Local Authority’s common application form.
