Supplementary information form

In addition to applying on the local authority (LA) application form, if your child requires special arrangements for the Academy's admission test, the Academy requires you to complete a supplementary information form (SIF). These supplementary forms seek information which is not collected on the LA application form, but which is needed by the school to administer the test. You can collect a SIF from the school or download it from the school's website. The SIF should be returned directly to the school and not to the Local Authority.

Please note that a SIF is only required if your child needs additional support for the Academy Test.
