Request support for an adult

Make a request for support to help you, or someone in need, live healthy, active and independent lives.

Eligibility

Adults may need care and support if:

  • their health has deteriorated and they need advice and support about what to do next
  • they struggle to look after their personal care and activities of daily living
  • they provide unpaid care for a friend or relative
  • they have a learning disability or physical disability
  • their mobility is compromised due to older age or other reasons
  • they have a hearing or visual impairment that affects their day to day life

Financial assessment

Requesting support means you may need to pay for all or some of your care. We will carry a financial assessment to help you understand how much you may have to pay. There are different ways of paying for your care. To find out more visit the paying for adult social care.
