What happens after the assessment

If you are eligible for support, we will look at:

• what support is available in the local community. A list of activities and support services are available on the  Simply Connect directory.
• the kind of support you need
• the types of equipment or adaptions you may need
• what support is available from your friends and family
• your own skills and resources

We will always support you to stay as independent as possible, living at home wherever possible and keeping the things that are most important to you.
