What information we need

We need some information to help us find the original registration for the birth, death, marriage, or civil partnership.

Birth certificates

To order a copy of a birth certificate, you will need to tell us:

  • full name at birth
  • place of birth (hospital or address)
  • date of birth
  • mother's full name
  • father's full name (if possible) 

Death certificates

To order a copy of a death certificate, you will need to tell us:

  • full name of the deceased
  • place of death
  • date of birth
  • date of death 

Marriage or civil partnership certificates

To order a copy of a marriage or civil partnership certificate, you will need to tell us:

  • full names of both partners
  • date of marriage or civil partnership
  • name of the venue where the ceremony took place
