If you are applying for either a new site or new mobile collector's licence please download the following forms:

There are 2 different fees for site licences and collector licences:
As part of the application a current criminal record check is required. This must be no more than 3 months old at the date of submission.
You should apply for your criminal record check through the Disclosure and Barring Service.
